What is the patient portal?
The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office. Patient Portal can also be used to communicate with the practice easily and safely.
How do I access the information in the Premier family portal?
In order to access the information, you will need to create an account by clicking the button below. You may also log into the portal by going to portal.premierfamily.net. When the account is created, you’ll be able to use the internet to perform tasks that used to require phone calls.

Where is my health information posted?
Your health information is posted to a highly secure data repository.
How to change follow my health settings?
Log into FollowMyHealth and from the left menu, go to the Preferences section and select the appropriate option.
What is proxy access?
Proxy access allows access to the health information of another family member or close relation.
Where does FollowMyHealth come from?
Your FollowMyHealth information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next office visit. Your health information is reviewed and updated in your electronic medical record each visit.
How long does it take to receive a follow my health email?
You will receive an email invitation within five business days. Look for ‘noreply@followmyhealth.com’ to activate your Premier FollowMyHealth® Check your SPAM filters if you don’t see it in your inbox.
What can be done within the document viewer?
The printing, faxing, and emailing of secure messages, scanned documents, appointment information and more can be performed within the document viewer.
