Late-night visitors must enter through the emergency department entrance on the south side of the building. The main entrance is locked from 9 p.m. – 5 a.m. Call the PVH switchboard at 970.495.7000 and request a specific department for more information, or to look up a guest.
If you have not supplied Pleasant Valley Hospital with your e-mail address, you must present a valid photo ID to the Medical Records Department, open Monday through Friday 7:30 am – 3:30 pm.
If needed, you may obtain a full copy of your electronic health record by contacting the Medical Records Department at 304.675.4340, ext. 1355. Certain documents will not be available for viewing via the Patient Portal.
Yes, your information is safe. Patient Portal passwords are encrypted, and URLs are rewritten so they cannot be copied and pasted. Only you and/or your authorized health care proxy have access to your Patient Portal.
Yes, this is called Proxy Access. Once a child turns 18, a parent can only access the child’s medical records with the child’s authorization . If you help manage the medical care of another adult, it is also possible to request access to that individual’s health record.

A list of your medicines (prescription and non-prescription) you are currently taking, along with their dosage amounts and when you take them. You may bring the bottles or containers. Include supplements, vitamins, and herbs you routinely take. This medication form can help you create an up-to-date medication list.
Meet with a pre-admission nurse who will explain what to expect the day of surgery and which currently prescribed medications can be taken before the surgery. You may also complete any required diagnostic tests after meeting with the nurse. Ask your doctor if you can eat or drink before your appointment.
The interfaith chapel at Poudre Valley Hospital is located on the first floor near the main elevators. As a place for quiet meditation and prayer, it is always open. Smoke & tobacco free campus. Smoking and tobacco products of any kind are not permitted anywhere inside or on PVH grounds.
Chaplains provide spiritual encouragement and counsel as well as compassionate emotional support for people of all faiths. Chaplain services. Call 970.495.7000 or you may ask a staff member to page the chaplain.
