San Luis Valley Health transitioned to a new patient portal experience with the new Electronic Medical Record (EMR), MEDITECH . Step One: Please contact Medical Records or any SLVH admissions representative (any clinic or hospital), and provide them with your email address, date of birth, and phone number.
Or you may send an email to patientportalsupport@slvrmc.org and provide your email address, name and DOB. Step Two: Check your email inbox and open the email from "Your Trusted Partner in Health." In that email, click on the link that says "Click here to sign in."
Visit the Medical Records Department at East Alabama Medical Center or EAMC-Lanier with your photo ID and a completed patient portal authorization form. A valid email address is required for accessing your patient portal.
