Follow the steps by entering in your date of birth and challenge answer given at the time of registration to create an account and get connected to your health information. After you have completed enrollment, log into the new Patient Portal. Patients will need to know their medical record number in order to self-enroll.

Dr. Jon C. Markley

Dr. Jon Markley is board-certified in physical medicine and rehabilitation and earned his undergraduate degree locally at Goshen College. He graduated medical school from Michigan State University College of Human Medicine in East Lansing, MI.

Dr. Tyler McGregor

Dr. McGregor is a board-certified orthopaedic surgeon. He was born and raised in Toronto, Canada. He attended The College of the Holy Cross in Worcester, MA, graduating in 2006 with degrees in pre-medicine and psychology. He was a 4-year member of the varsity hockey team.

Dr. A.J. Mencias

Dr. Mencias is Physician President of South Bend Orthopaedics. He was born in Louisville, Kentucky and then eventually moved to the Indianapolis area while he was still young. His parents are first generation immigrants from the Philippines.

Dr. Brian Ratigan

Dr. Brian Ratigan is a Board-Certified Orthopaedic Surgeon who specializes in sports medicine with an interest in arthroscopic and reconstructive surgery of the shoulder and knee. He is the team physician for Notre Dame football and baseball.

Dr. Deepak Reddy

Dr. Reddy grew up in Chicago. He is a fellowship trained Orthopaedic Spine Surgeon specializing in Adult Complex Spinal Surgery/Revision Spinal Surgery, Minimally Invasive Surgery, Robotic Spinal Surgery and Cervical Spinal Surgery including Disc Replacement.

Dr. William Rozzi

Dr. Rozzi was born and raised in Northern Indiana and was educated at both Purdue and Indiana University. He has been in practice in South Bend since 1990.

Dr. James Sieradzki

Dr. James Sieradzki is specializes in sports medicine, arthroscopic surgery, and joint replacement surgery. He is the Team Physician for Notre Dame Hockey and Notre Dame Lacrosse.
