How do I access my health information at Ascension Seton?

At Ascension Seton and Dell Children’s Medical Center, we make accessing you or your child’s health information convenient with our patient portal. You can view information from your visit at Ascension Seton or Dell Children’s Medical Center through our patient portal.

Since 1995, SETMA has grown from five providers in one location to 45 providers in six clinical locations. Whether provider, patient or employee, our SETMA team is here to make your life and your health better.

How do I enroll in the my Seton health portal?

To enroll in the My Seton Health portal, which contains your hospital care information, please fill out the appropriate form below and bring it, along with a government-issued ID, to any Ascension Seton or Dell Children’s location. You will then receive an email to complete your registration.

What is Medicare Welcome to Exam?

Your "Welcome to Medicare" Exam is a preventive visit within the first 12 months of Part B coverage. This visit is covered by Medicare. If you have further questions, you can visit the Medicare website or feel free to call our billing department at 540-678-2837.

How to allow someone else access to my medical records?

To allow anyone other than yourself access to your records or make requests, you must fill out an. Authorization for Disclosure of Health Information form. , listing anyone you wish to have access to your account, and bring the form to Selma Medical or fax it to our Medical Records department at 540-678-2859.
