Browser Requirements. Beginning on November 12, 2020, South Shore Health will no longer allow connections to South Shore MyChart from older/legacy web browsers.
Pediatric and Disabled Teen proxy access allows you to securely communicate on behalf of a pediatric patient who is under the age of 13 (or 13 -17 years if the child is disabled). Access is obtained by completing the Pediatric and Disabled Teen Proxy Authorization Form. When the patient turns 13, this will transition to “Adolescent Proxy” access, with the ability to view a limited portion of the patient’s records.
We offer our patients secure internet access to their own medical information on line, so they may view their personal health record at any time or place with internet access.

Shore Neurology, PA is a participating provider for Medicare and most major insurance plans. We are happy to process all insurance forms for you as long as we are provided with complete and accurate information about your insurance coverage.
We see patients by appointment only. Although we work very hard at running on time, and even though we leave time in our schedules to accommodate delays, we will sometimes be unavoidably late and we apologize in advance.
Requests for prescription refills are renewed during normal office hours and may be made through our Patient Portal. In some instances, we may require that the patient be seen in our office prior to the medication being refilled. Please allow at least 48 hours during our normal business hours for your request to be processed.

His postgraduate training included a medical internship and neurologic residency at Sloan Kettering Memorial Hospital and North Shore University Hospital, where he was the Chief Resident. Dr.
Dr. Gudesblatt, the medical director of the Comprehensive MS Care Center at South Shore Neurologic Associates, P.C., graduated from Johns Hopkins University as well as from Cornell University Medical College.
Dr. Rosen graduated from Johns Hopkins University School of Medicine in 1985. Postgraduate training included medical internship at the University of Maryland and neurology residency at Johns Hopkins Hospital. He received his M.B.A. in Healthcare Administration from Regis University in 2005. Dr.

Dr. Xian graduated from Lanzhou Medical College, People's Republic of China in 1982 and received his Ph.D. degree from the University of Minnesota Medical School in 1992.
