How do I sign up for my St Jude?

Sign up by filling out the form on the last page of this brochure. Tear off the form and hand it to a Patient Registration staff member or your patient representative. You can also place it in one of the My St. Jude boxes located in the clinic area.

What information can I see about my child on Myst Jude?

My St. Jude allows you to see your (your child’s) personal information such as address, phone number, and insurance. You can send a request to the Patient Registration staff to update this information. You are not required to sign up for My St. Jude.

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
