Hawthorn’s patient portal, StewardCONNECT, is now active. The portal allows you to connect securely with your doctor and medical team online, and to view and verify your health information.
Steward Medical Group and Hawthorn Medical are providing patients with another way to access their medical records with their Apple iPhone Health app. This enhanced access allows patients to easily view key parts of their records, including allergies, conditions, immunizations, lab results, medications, procedures, and vitals.

My Health Home patient portal is a free and secure online home for your inpatient hospital health information. The portal allows you to view your medical records on any internet-enabled device and become a more active partner in your health care.
All health data is stored on a secure server and managed with the protection of your information in mind. Only you – and those you authorize – will be able to see or gain access to the information in your Portal.
After you’re discharged from the hospital, you can easily access discharge instructions and other information to help with your recovery. With proper authorization, you can manage a family member’s health records in his/her Portal account. Understanding and using your hospital Portal account will help you.
