What is the mystjohnshealth portal?
Thank you advance for your interest in St. John’s Riverside Hospital’s MyStJohnsHealth Portal, a web-based patient portal that provides you with secure and convenient access to your health information. Please complete the “Request for Access Form” below and click submit.
Can I use mystjohnshealth to talk to my doctor?

How to upload a form to a website?
To upload your complete forms: Click on the “Upload” tab. Review the Instructions provided. To the bottom of the page, select the type of document you will be uploading, e.g., Immunization History. Click “Select File” to browse your computer for the document. Upload.
Do you have to log out of the patient portal?
As with any other personal identifying information on the Internet, always log out of the Online Patient Portal and close your internet browser when you are done using the portal to ensure your medical information remains confidential.
How to contact the Health Information Services Department?
If you are sure you have entered all the characters exactly as they appear in the email sent to you but are not able to log on, contact the Health Information Services department at 914.964.4500 or send an E-mail.
When can a parent access a child's medical record?
Per State and Federal guidelines, after the child reaches age 12, parents/guardians will need an additional authorization signed by the child to have access to the child’s medical record information. Once a child turns 18, a parent can only access the child’s medical record information with the child’s authorization.

Who can access your account?
Rest assured your information will always be kept confidential between you and your Provider. However, you can choose to give another person access to your Ascension St. John Patient Portal account. They will be called your proxy. If you are in person at your clinic, you can fill out the required paperwork to add a proxy.
