Under the user name and password field is a link…. “Need help with your user name and Password?” Click on this. You will then be asked for your user name or personal information (which must match our office records).

Uninsured patients are offered discounted services through an income verification process. TFHC accepts Medicare, Medicaid and various private insurance plans. Please click here for a complete listing of payment options.
Please download and bring the form to our office if you need assistance filling out. If you have no printer a copy will be provided for you at our facilities.
