As we continue our efforts to combat the effects of COVID-19, the safety of our patients and staff is of upmost concern. Therefore, we are not allowing in-person access to our Release of Information department, with the one exception noted below.

Radiology Images and Studies

Radiology images and studies require one full business day to prepare. If you are scheduling follow-up appointments and will need these materials, please plan accordingly.

Medical Record Delivery Options

After we have processed your request for medical records, we will mail the information to the address specified on your request form, or you may pick up your copy from our office. Due to the large volume of requests we receive, it may take up to 5 business days to fulfill your request from the date we receive it.

Why have we revised our visitation policies?

Consistent with city and county efforts around social distancing to limit the exposure to individuals who may have contracted COVID-19 but are at that time asymptomatic, we have revised our Visitation Policies to help protect our patients and their loved ones.

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
