The Little Clinic provides high quality, affordable healthcare located in convenient retail settings. The Little Clinic diagnoses and treats minor illnesses for patients 12 months and up, with care provided by board-certified nurse practitioners and/or physician assistants.
Our History
The Little Clinic began in 2003 with its first locations in Louisville, Kentucky. Company founders believed a new model was needed to make quality healthcare more accessible and affordable for busy individuals.

How do I obtain my medical records? To request a copy of medical records, please complete the Authorization Form. Email the completed form to medicalrecords@thelittleclinic.com or fax it to: 615-425-4344.
