How do I sign up for the patient portal?
During your visit, let the clinical team know you would like to sign up and they will help you complete the Patient Portal registration process. If you prefer, you can also ask for an activation code during your visit so you can set up your Patient Portal account at home by clicking the above link to begin the activation process.

We support a healthy lifestyle by providing high-quality, comprehensive and affordable primary health services to all, regardless of a patient's ability to pay. For those who are uninsured or underinsured, we offer a sliding fee discount program, allowing us to "slide" or reduce fees for eligible patients.

We offer a patient-centered, medical home model approach to primary care, meaning families can receives medical, dental, behavioral health and recovery services, all under the same roof and led by a primary care team.

Shaping the future
We are a Teaching Health Center, which means patients will see a resident physician as part of their care team when visiting one of our locations. A resident physician holds a medical degree and works under the supervision of an attending physician.
What is health records online?
Health Records Online enables you to manage your health anytime using your computer, smartphone or tablet. Connect with your doctors no matter where you are, at home, around town, or out of state.
Can you share your health records with other providers?
No. Only you may elect to share summaries of your care with other healthcare providers. We do not share your personal information and strictly follow our Privacy Policy. You may read that policy online or when you enroll in Health Records Online.
