Our Story

We began in 1996 as a Rural Health Center in Trafalgar, Indiana - born out of a shared vision by citizens of the community and leadership from Johnson Memorial Hospital to offer primary medical care for people in Southern Johnson County.

Note: HIP and Medicaid patients must be assigned to a WHN provider to establish care at our office.

What is a telehealth visit?

This is a real-time, virtual visit with your provider simply using your existing mobile device or computer, from the comfort of your home, or wherever you may be. Depending on your needs your provider will determine whether a telehealth visit is right for you.
