Resetting your password is simple. Just visit the log-in page for YRMC CareConnect and select the link “Forgot Your Password.”. Then follow the instructions to reset your password. If you continue to have trouble logging in, call 1-877-621-8014.
How old do you have to be to enroll in YRMC CareConnect?
Parents and legal guardians can also enroll to create accounts in YRMC CareConnect for each child up to 12 years of age. When the child reaches 12 years of age, the account will be discontinued. If at any time you require technical assistance for YRMC CareConnect, please call 1-877-621-8014.

Health Information Management (HIM) – also called Medical Records – manages patient health records for Dignity Health, Yavapai Regional Medical Center (YRMC), Yavapai Regional Medical Group, Prescott Valley Medical Imaging and Prescott Medical Imaging. Our HIM team is committed to protecting and maintaining health records to support the delivery of patient care. The HIM team: 1 Manages all YRMC-related inpatient and outpatient health records 2 Provides information for birth certificates and death certificates 3 Assists with amendments to health records 4 Oversees the appropriate release of information related to care provided to YRMC patients at all YRMC inpatient and outpatient locations—YRMC East, YRMC West, Yavapai Regional Medical Group, YRMC Del E. Webb Outpatient Center, YRMC Outpatient Services Building, the YRMC Wellness Center, Prescott Valley Medical Imaging and Prescott Medical Imaging 5 Helps patients set-up their personal accounts to access their health records online through YRMC CareConnect
What to do if you review your medical record and believe it is incorrect?
If you review your health record information and believe that information is incorrect or incomplete, you may ask YRMC to amend your record. YRMC asks that requests for amendments to medical records be submitted in writing to: Amendment of Medical Information.
