How do I login to the patient portal?

At the hospital or practice registration desk (where you “check-in”), ask the registrar to activate your Patient Portal. You will be sent an email with a link to the patient portal (hospital or physician practice portal) and directions on how to set your password. Click on the link, follow the directions to set your password. Log out of the portal.

I’d recommend anyone with a medical equipment need to Cooley. What a terrific group. The company and the people are incredible, they were absolutely amazing with supplying my grandmother oxygen and her bipap along with all the supplies. They were reliable and courteous.

How do I access the “mycarecorner” portal?

Once you have set up your “MyCareCorner” portal you will access the portal for future use by going directly to the MyCareCorner site at MyCarecorner.net MyCareCorner will provide you with your health information and more. At the hospital or practice registration desk (where you “check-in”), ask the registrar to activate your Patient Portal.

Local, friendly staff. Always willing to go the extra mile to help. If you need to go into the store my tip is to park behind the store on Market street and enter thru the rear, there is a sign. It is much easier than parking on Park Ave. They will take great care of you.

Patient Portal

We have a new patient portal, and you can log into the patient portal by clicking on the SIGN IN button. If you have never logged into the Patient Portal, please call our office at 972-758-5484, and we will send you an email with credentials and instructions on how to use the portal.

Insurance Information

Our staff is happy to offer professional assistance in filing your insurance. Dr. Cooley participates with most major insurance plans including Aetna, Blue Cross, United Healthcare, CIGNA, Medicare as well as many other preferred provider insurance plans.

Procedures

Dr. Cooley performs procedures at Endoscopy of Plano located within Medical City Plano hospital in Suite 401A.

Billing

As a member of the Digestive Health Associates of Texas, Dr. Cooley’s billing is handled by the central billing office.

Our Prescription Policy

For prescription refills please contact your pharmacy and have them call our office at (972) 758-5484 to refill your prescription.
