If you are a new or established patient of Core Physicians, you can choose one of three ways to enroll in the Patient Portal: 1. Visit any Core Physicians’ office 2. Call our Customer Service line at 603-580-6753
How do I request enrollment in the patient portal (nextmd)?
New and established patients of Core Physicians can use this form to request enrollment in the Patient Portal (NextMD). Once we receive your completed form and after validating your information, we will notify you of your temporary enrollment token number.
How do I contact the core Institute for medical records?
You can contact our Document Management Department if you have any questions at Fax.Main@thecoreinstitute.com regarding your medical records. The CORE Institute is dedicated to providing our patients with superior patient care.

To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them.
