In order to protect our community and prevent the spread of infection, Covenant HealthCare has implemented virtual office visits with the majority of Covenant Medical Group primary and specialty care providers. Virtual office visits allow patients to maintain regular appointments with Covenant Medical Group providers without having to physically go into an office.

After submitting your general demographic information, you will automatically be directed to the Universal Provider Profile and Pre-Application.

Step 3

After submitting the completed Universal Provider Profile and Pre-Application, it will be reviewed. Note: You must currently hold a license to practice medicine and maintain a federal DEA registration, if applicable.

Step 4

You will be contacted by a Covenant Health representative to guide you through the next phase in the process.
