How do I login to my Patient Portal?

In order to begin using your portal you will need to provide us with your email address. At that time you will be given a username and password. The first time you use this site, you will be allowed to create your own security password. You will be notified to go to your patient portal by an email from East-West Health Center This is a secure site.

Do I have to be a Denver Health patient to access MyChart?

You do not have to be a Denver Health patient to access your child’s MyChart account. Ask a staff member to create a MyChart account as a non-patient to view your child’s medical information. Learn more about your options for you child's MyChart access on our frequently asked questions page .

Can you verify that I have access to the patient portal?

We can verify that you have access to the portal and that we have your correct email. We must have a valid social security number to grant you access to the patient portal. DOB format must be MM/DD/YEAR & phone number format must be 2223334444 (no special characters)

Sign Up for a New MyChart Account

You will need your activation code, which can be found on your enrollment letter, bill or the last page of your visit summary. (If you do not have an activation code OR your code no longer works, click on “ Request Online .”)

In addition, your provider will communicate with you via this portal to relay results of laboratory tests, radiologic studies and other valuable instructive information.

A SECURE PLATFORM

This is a secure site. Should you ever have difficulty with this process, please call our office and someone will either reset your password or assist you with the process.

Office Hours

If you need us before or after our regular office hours due to an emergency situation, please call 911. If you need us for a non-urgent matter that does require after hours attention, please call the office closest to you to be connected to the on-call physician.

Arriving For Your Appointment

New patients are asked to arrive 30 minutes early to complete paperwork. You can download and print the patient forms from each practice's website and complete them prior to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We are happy to help with refill requests during regular office hours, so please call your doctor's office to let us know what you need.

After Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call the closest office to you and our answering service will assist you.
