Your patient portal, known as My FMC Portal, will help you manage your health and wellness by keeping you connected in a secure environment to access records and other information related to your healthcare.

What is a medical release authorization form?

A Medical Release Authorization Form allows you to request copies of your medical records, lab work, imaging results, and more. Visit this page to download a copy of the form, and for information on where to send completed paperwork and requests.

What is health information exchange?

Health Information Exchange is an automated system that allows your health information to be shared among physicians, hospitals and other providers. The HIE gives your providers access to information necessary to treat you, reduces the need for duplicate tests and treatments, and reduces the chance of medication errors.

Where to fax medical records?

If you wish to personally pick-up the copies, please bring a photo ID (driver’s license, state ID, employment badge, etc.) to the Medical Records Department, located on the first floor at Fairfield Medical Center.

Can you fax a medical record to a physician?

Include the phone number and fax number, if you wish the information to be sent to a physician. We typically only fax records to physician’s offices, if they are needed sooner than can be sent by mail.

Price Transparency

Hospital price transparency helps patients know the cost of a hospital item or service before receiving it. Starting January 1, 2021, each hospital operating in the United States will be required to provide clear, accessible pricing information online about the items and services provided.

How to enroll in the patient portal?

To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.

How are messages sent to your provider's office transferred?

All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.

Can you access read only medical records?

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records. Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.

Is RWJBarnabas Health a centralized portal?

RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
