If you are a new patient making your first appointment with Lexington Clinic, please come to the office 15 minutes prior to your appointment time to fill out paperwork. If you have insurance, please bring your insurance card.
If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to your local emergency room, and/or call your physician. MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record.
