New Patients

If you are a new patient making your first appointment with Lexington Clinic, please come to the office 15 minutes prior to your appointment time to fill out paperwork. If you have insurance, please bring your insurance card.

What to do if you need medical emergency?

If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to your local emergency room, and/or call your physician. MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record.

Does Lexington Medical Center respond to messages?

Lexington Medical Center will respond to your message in as timely a manner as is reasonably possible. It is your sole responsibility to monitor your account for messages and other information and to respond and /or act (or not) upon them in a timely and appropriate manner. IV.

Do I need to use MyChart to get medical care?

If you do not wish to be bound by these terms and conditions, do not access or use MyChart. The use of MyChart is completely voluntary and is not required to receive health care from Lexington Medical Center. Individuals must be age 16 or older to request a personal MyChart Account.

Can you disregard medical advice?

Never disregard medical advice or delay in seeking it because of something you read in MyChart. NEVER USE MYCHART FOR EMERGENCY OR OTHER URGENT MEDICAL ISSUES. If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to an emergency room, and/or call your doctor. VIII.

Lexington Medical Center allows you to view or download a single copy of the material on MyChart solely for your personal, noncommercial use.
