We are conveniently located at 804 English Road, Suite 100 Rocky Mount, NC 27804. Adobe Reader Software is required to view some of these documents. How do I access my online medical records?

Once you are registered with our office, you will be able to access your online medical records through our eClinical software. You must provide us with a valid email address to access this information.

Our Patient Portal Gives You Secure, Online Access to Your Health Information for Visits to Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT)

Log in to the Patient Portal to quickly and easily access your health records. If you have not been given access to the Patient Portal, please contact your physician’s office. Access will occur upon your first visit to the following clinics: Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT).

Authorization for Release of Information

Please download the form, fill out and return to the Aspen Valley Primary Care clinic prior to your upcoming appointment. The form can be taken to either Primary Care location in Aspen or Basalt, or faxed to the Primary Care clinic at 970-927-3915. Quality Improvement Project Title: Improving Patient Portal Enrollment Through Education and Technology at a Rural Family Practice.

Who is Karen Locke?

Dr. Karen Locke has been practicing medicine in the Roaring Fork Valley with her husband, Dr. Kelly Locke, since 1999. In 2019, the Lockes partnered with Aspen Valley Hospital to start Aspen Valley Primary Care. Dr. Locke studied Biology at Colorado College before earning her medical degree from the University of Kansas. She completed her residency at the University of Wisconsin, Eau Claire Family Medicine Residency, and is board certified by the American Board of Family Medicine. Before moving to the Roaring Fork Valley, Drs. Karen and Kelly Locke lived in Germany, where he practiced for the U.S. Air Force and she treated civilians through the American Red Cross. Dr. Locke sees patients of all ages, from infants to the elderly. Her areas of interest include women’s health, adolescents, hormones, and mental health issues. Dr. Locke feels fortunate to have raised her family in the Roaring Fork Valley where they can enjoy the mountains, the seasons and the people who call this place home.
