The My Lodi Health patient portal will continue to display past medical records but will no longer provide access to message your provider or view new records. All new medical records can be viewed on your new MyAdventistHealth account when you sign up after an appointment with your provider.

What is the legal age to release medical records?

Unemancipated minors under 18 must have a parent or legal guardian provide a written authorization for release of medical records. For medical records of a parent or spouse, that individual must provide written authorization. If the parent or spouse is not able to sign, authorization from a legal representative of the patient may be accepted.

How long does it take to get a patient's medical information?

Patient requests for information are generally completed within three to five days from the date of the request.

Who can register?

If you’re a patient at LMH Health or one of our affiliated clinics and are 18 or older, you can register online for My Patient Portal. Parents also can also register their children who are younger than 13. Learn more about how to register.
