The Health Records Online patient portal is your personal view into the electronic medical record that your healthcare providers use to manage and document your care. How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

How do I create an AllianceHealth Durant and Madill account?

You can create and access your account via the AllianceHealth Durant and Madill website or by downloading the MyHealthHome – MphRx mobile app available in the Apple App Store or on Google Play.

How to grant access to MyHealthHome?

To grant a caregiver, guardian or trusted family member/friend access to your MyHealthHome account, the requestor will first need to create their own MyHealthHome account. Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.
