How do I change or Reset my Patient Portal password?

Please contact your medical provider’s office for instructions on how to change or reset your patient portal password. Once you can log in to your patient portal, you’ll be able to make changes to your security question or access code.

As you arrive for your first appointment, we would like to ask that you arrive at least 15-20 minutes before your appointment. This gives us time to input your insurance card, new patient forms, patient information and more. Additionally, please make sure to bring a list of any medications that you are taking currently.
