MySite Patient portal is a free, secure online resource that gives you easy access to your health records. To continue accessing your health records for visits prior to October 1, 2018, you must log on or enroll in the ORIGINAL MySite account.
Saratoga Hospital Medical Group’s Patient Portal is a secure website that allows access to your medical information. Through this Patient Portal you have real-time access to your medications, labs, and upcoming appointments.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
If you contact Primary Care of Milton using MyOneChart and do not receive a response within 72 hours, please call our office directly at 470-639-6340. MyOneChart is your new patient portal. MyOneChart is a free, secure, web-based system that allows real-time access to your personal health information and medical records.
Primary Care of Milton is a member of a network of physician practices that utilizes MyHealthHome, a secure, web based system that allows you to contact our doctor electronically. MyHealth Home is available 24 hours a day for you to review your labs and conduct non-urgent communications with our office.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
