What is the patient portal and how do I access it?

The MRMC patient portal is an electronic health record that includes a view of your clinical data. The data on the portal includes test results, medications, allergies, immunizations, health issues, and billing statements. How do I access the patient portal once I have completed my initial account set-up?

How do I contact the clinic portal support center?

Looking For the Clinic Portal Instead? Clinic patients should call (979) 241-6100 for support. Prefer A Text-Only Patient Portal View? Forgot your Username and/or Password? The text-only version allows patients with certain disabilities to more easily navigate and access their health care data.

How do I contact KRMC medical records?

If you have questions or need assistance in using either portal, please contact KRMC Medical Records at 928-263-4500.

How do I sign up for an evident patient portal?

You will receive an email invitation (from donotreply@evident.com) to create your patient portal account. Click on the link provided in the email to start the registration process. Enter first name, last name, and date of birth.

ENROLL ONLINe

Once your name, social security number, date of birth and email is on record in the current medical record system (launched November 2018), you may self-enroll if you are 14 years of age or older. To start the self-enrollment process, click here .

ENROLL at your next visit

At the time of registration, you will be asked if you would like to enroll in the portal and a registration staff member will complete the first step for enrollment, asking you to provide basic demographic information and the answer to a challenge question (e.g. mother’s maiden name).

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Moberly Regional Medical Center will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Note to previous Portal Users

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

If at any time you require technical assistance for YRMC CareConnect, please call 1-877-621-8014. Please note that some older Internet browsers may not function properly with YRMC CareConnect. If you do experience compatibility issues, you may need to upgrade your browser.

How to reset YRMC password?

Resetting your personal password is easy. Just visit the YRMC CareConnect sign-in page and click the link for “Forgot Your Password.”. Then follow the instructions to reset your password. If you are still having trouble signing into the site, call 1-877-621-8014 for assistance.
