Welcome to my MVH record Patient Portal, which offers you secure, easy, free access to your Monongahela Valley Hospital health care records over the Internet. View, download or transmit your personal health records from your visits to MVH and offsite outpatient departments

Where is outpatient registration located at MVH?

Outpatient Registration is located on the first floor of the hospital, beyond the Concierge Desk. Beginning October 1, 2019, MVH will allow patients to access their health information through an application programming interface (API).

When will the new health patient portal be available?

The NEW Health Patient Portal only includes medical information that has been added after October 14, 2019. For information prior to October 14, 2019, please use Previous Patient Portals. Self-enrollment is available for Mid-Valley Hospital & Clinic patients who are 13 years or older. Click HERE to self-enroll!

Having a patient portal account helps the practice create a more accurate record of your medical history as well as educate you on certain aspects of your health. It provides you and the practice with a more efficient means of communication, eliminating sources of frustration such as phone tag.

Patient Portals

Mohawk Valley Health System (MVHS) MyChart allows for secure online access to your electronic medical record (EMR). Important health documentation from your doctor is stored in MyChart and patients are able to find health information such as lab results, appointment information, medications, immunizations and more.
