A myKSHealtheRecords personal health record (PHR) is a smart way to manage your medical information in one online location. This PHR will allow you to access your records any time you need them - everything from medications and allergies to previous illnesses and injuries.

What is a participating provider?

Participating physicians, hospitals, and other healthcare-related entities, who all play a part in caring for you, share your current health information through a secure, electronic health information network. Your participating healthcare providers have access to your full patient record to include diagnosis, treatment, lab results, medications, etc., in an effort to deliver you the most efficient and thorough services possible.

MyChart allows you to quickly and securely pay your medical bill online.

What is my chart?

MyChart offers secure , online access to portions of your electronic medical record (EMR). It allows you to schedule an appointment, request a prescription refill or ask your provider a question. You may also conveniently view your medications, immunizations, allergies, health information and lab results. You can join a waitlist to receive notification of earlier appointments and can prepare for visits in advance by completing health questionnaires online. You can pay bills, authorize a copay and get cost estimates for care you may be planning to receive.

What is Patient Portal?

CHC/SEK’s Patient Portal is a secure website that allows patients to view their personal health records, including labs, diagnostic imaging, immunization records, upcoming and historical appointments, visit summaries, and education. You can also view questionnaires, messages from staff, billing statements, and demographic information.

How to reset patient portal password?

Not a problem! You can reset your password on the Patient Portal login page by clicking ‘Trouble logging in’ and selecting the ‘Forgot my password’ option. You just need to answer your security question and then you’ll receive an email with your login details. If you still can’t log in, call us at 620-231-9873 and our friendly staff will assist you in resetting your password.

At Community Health Center of Southeast Kansas (CHC/SEK), we want you to have easy access to your health information. One way we do this is through Patient Portal, which provides convenient 24/7 access through a secure, password protected website.

If you need technical support, call 877.621.8014. myhealth is not intended for use in emergencies. For urgent medical matters, contact your physician’s office, go to the emergency room or call 911. If you do not have a physician, you can use our free find a doctor search or call 816.221.HEAL (4325).

What age do you need to be to create a patient portal account?

Active email account; Please note: Each family member age 18 or older needs to have a unique email address to create a patient portal account.

The my health patient portal is free and available to all patients. There are two ways to sign up.

myUHealth is an online secure tool provided by University Health to our patients to help them better manage their care and improve their overall healthcare experience. myUHealth can be accessed 24 hours a day, 7 days a week, 365 days a year from any device (computer, tablet, smartphone, etc.) with an internet connection.

How long does it take for University Health to get in touch with you?

We will be in touch typically within two business days with instructions on next steps.

For patients who are: 1. Discharged from the Emergency Department or hospital inpatient areas while your COVID-19 test results are pending or 2. Tested at our drive-up testing facility
