If I cannot log in to my MyChart account after trying a few times, what should I do? To protect your health information, your MyChart account will lock after five incorrect login attempts. Please call the MyChart help line at (312) 563-6600 630-724-5080 between 8 a.m. and 6 p.m., Monday through Friday. A help line representative will unlock your account and show you how to reset your password.
To use the mobile MyChart app, you must first have a MyChart account. To get the app, you need an iPhone®, iPadtm, or iPod touch®. From one of those devices, go to the App Store application and install "Epic MyChart". From and Android device, go to the Market application and search for "Epic MyChart".
MyChart is EPIC's patient portal. MyChart is a secure means for patients to access parts of their health records through a web browser and cell-phone apps.

The MyChart patient portal solution, by Epic EHR, provides a standard array of patient portal features such as allowing patients to access their medical records, message their doctors, access patient education materials, attend e-visits, request medication renewals, complete questionnaires, and schedule appointments.

If you still have your most recent After Visit Summary, your activation code is printed at the bottom. If you don't use your code within the 60 day period, you'll need to ask for a new one. Call 1-855-513-5513 and a MyChart representative can provide a new activation code.
If you have an e-mail address on file then your MyChart Username will be sent to your e-mail account. If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 866-385-7060 to help you regain access to your MyChart account.

If you aren't currently signed up for MyCare, you will need to set up your MyCare account with your information and a personal activation code that you may have already received. If you haven't been given a personalized code you can request one, and then continue with creating your account.
If you're new to MyCare and have not received an enrollment letter from us, you can request an activation code here. You will be prompted to answer a series of questions to help us verify your identity. If you are under 18 you must contact your Primary Care Physician to activate your account.
