Where can I see an urgent care provider in Riverside?

You'll see one of our experienced urgent care providers from M.D. Express, part of Riverside since 2011. The on-line clinic is open seven days a week, 9 a.m.-8:00 p.m. Visits are open to teens and adults, ages 13 and up. Children ages 5-12 may be seen with a proxy adult.

When can a parent access a child's medical record?

Per State and Federal guidelines, after the child reaches age 12, parents/guardians will need an additional authorization signed by the child to have access to the child’s medical record information. Once a child turns 18, a parent can only access the child’s medical record information with the child’s authorization.

Riverside Community Hospital is conveniently located off the 60/91 freeway in Downtown Riverside.

Parking

Riverside Community Hospital offers complimentary valet parking to our patients and their guest between the hours of 8:00am - 5:00pm Monday - Friday. Additional free self-parking is available located adjacent to the main hospital entrance or in our parking structure located off Brockton Ave.

Patients should enter Riverside Community Hospital through the main entrance located off 14th and Magnolia Ave.

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests.

How to obtain a copy of medical records?

Please complete the following steps to obtain a paper copy of your medical records: 1 Print and complete the Medical Records Release Form. 2 Complete, sign and date the form. In order to verify your identification and validate your authorization, we require that you include a legible copy of a valid photo I.D. (e.g., driver’s license, military I.D. or state I.D.). 3 Mail, fax, or email your request.
