How do I view my Shore Medical Center Records online?

All Patients Must Create A New Account! Shore Medical Center launched a new patient portal on August 28, 2018. If you have not yet done so, you will need to sign up for the new portal in order to view their Shore Medical Center records online at your convenience. The portal that was being accessed prior to August 28, 2018 is no longer in use.

What services does shore Physicians Group provide?

Shore Physicians Group provides many additional services for patients in our community. Shore Physicians Group hospitalists staff Shore Medical Center 24 hours a day to care to patient who are admitted to the hospital.

How many visitors per patient?

One visitor at a time per patient will be permitted. Visiting hours are as follows:

How old do you have to be to visit a hospital?

Upon entering the hospital, you will present your ID and undergo a symptom and temperature check. All visitors must be 18 years of age or greater, with rare exceptions. You will receive specific guidelines for visitation from the front desk staff.

Do you have to wear a mask in a hospital?

All visitors must comply with the requirements to minimize spread of infection, including practicing 6-feet social distancing and wearing a surgical mask while inside the hospital as recommended by the CDC. (no bandanna, gaiters, or masks with exhalation valves as a mask). A surgical mask will be provided to visitors if necessary. If a visitor refuses to wear a mask or other PPE as indicated, they will be asked to leave the hospital.

Is Shore Medical Center tobacco free?

Towards that end, the Shore Medical Center campus is tobacco-free. Use of tobacco is not permitted in the facility, parking areas or adjoining spaces. We respect our patients' right to privacy, confidentiality and to fully understand the details of their condition and treatment options. Shore Medical Center has staff translators and a language translation phone service to ease communication for those who do not speak English. This will help insure that all patients and their families understand important healthcare information.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – ShorePoint Health Port Charlotte and Punta Gorda will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Note to previous Portal Users

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.
