How do I sign up to use the patient portal?

If you are an existing patient and would like to use our patient portal, please call our office at (817) 540-4477 to request an invitation to register.

WTO physicians and staff provide a comprehensive scope of orthopedic care that includes arthroscopic joint surgery, shoulder surgery, hand and upper extremity surgery, foot and ankle surgery, total joint replacement and sports medicine.

Patient forms

In order to expedite your appointment and office visit, it is advisable to print the forms above and have them completed accurately upon arrival to your appointment. We suggest that you arrive approximately 30 minutes before your scheduled appointment and be prepared to provide us with your insurance card, identification and any necessary co-pays.

Payment methods

We gladly except cash, personal check, Visa, MasterCard, American Express, Discover, Health Savings Account debit cards and Flex Spending Account debit cards.

Telephone Calls

If you have a medical question during regular office hours, please call the office of your physician. Your questions will be answered as soon as possible.

Prescriptions

Prescription refills must be requested weekdays only during normal business hours. Refill requests cannot be accommodated outside of regular office hours.

Billing Inquiries

Our Billing Office staff with gladly assist you with any questions or concerns that you might have about your account.

Appointments & Consultations

All appointments are scheduled in advance with the physician of your choice by calling Monday through Friday. Please call our office for additional information.

Cancellations

If you are unable to keep an appointment or need to reschedule, we ask that you kindly provide us with at least 24 hours notice. If not, a cancellation fee of $50 will be charged.
