What is patient fusion and how does it work?

Patient Fusion helps you keep track of your health records, manage your appointments, communicate with your doctor and manage your health spending. If you don’t have access yet, contact your doctor's office or find a new Patient Fusion doctor today.

How do I create an account with patient fusion?

As soon as your doctor enrolls you in Patient Fusion, you should receive a registration email containing a link that you'll need to create your account. If you did not receive an email, check your spam or junk email folders.

How do I Reset my Patient fusion password?

To reset your password, go to www.patientfusion.com and click “Forgot Password?” to reset your password. You will be prompted to enter your username. You will then receive an email with instructions on how to reset your password at the email address your doctor used to create your Patient Fusion account.

How can a patient fill out intake and registration forms?

Patients can log in and fill out customizable intake or registration forms from their patient portal and the information will flow directly into their chart note — without the need for double entry.

How do I book an appointment for my child or someone under 18?

Currently, patients must be 18 or older to request an appointment online. If you need to request an appointment for a minor, please reach out to the doctor's office directly.

How can I get a list of my current medications?

Your current medication list will appear in the Heath Records section under the Medications tab.
