We are excited to present Premier’s online Patient Portal as another step towards creating more efficient and effective communication between you and our office. The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office.

Family Practice, Internal Medicine, Pediatrics and Geriatrics are available, giving you all the options you need for your entire family within the same medical group. Premier Health Associates offers fourteen different specialists representing ten different specialties of care.
How do I register for an account with Premier Medical Associates?
To register for an account, please speak to our front desk staff at any Premier Medical Associates location or request an invitation online. Present your photo ID at any Premier office. Our EHR staff will be notified that you’ve signed up once your identity is verified. You will receive an email invitation within five business days.
How do I Activate my Premier FollowMyHealth® account?
Look for ‘noreply@followmyhealth.com’ to activate your Premier FollowMyHealth® Check your SPAM filters if you don’t see it in your inbox. Your email invitation will include a link to the Premier Medical Associates FMH login page. Select the green "FMH Secure Login" icon and create a username and password for your account.
